If you are bringing your pet directly to AVHS following euthanasia, you may complete the Public Cremation Form below before your arrival, or we will be happy to assist you in completing the form when you arrive.

Please Note:

AVHS is honored to partner with most veterinary clinics throughout the Arkansas Valley region to provide cremation services for their clients. If your pet will be staying with one of our veterinary partner clinics, you do not need to complete the Public Cremation Form.

Your veterinary clinic will complete and submit a cremation authorization form on your behalf. Because families sometimes change their selections during this difficult time, AVHS relies on the cremation form submitted by your veterinary clinic as the final authorization. To ensure your wishes are honored, please review your cremation selections with your veterinary team before the form is submitted.

If you have any questions about our cremation services or need assistance, we’re here to help. Please call us at 719-395-2737.

Thank you for trusting us to care for your beloved companion.

RAINBOW BRIDGE

It is called the Rainbow Bridge because of its many colors.
Just this side of the Rainbow Bridge there is a land of meadows,
hills and valleys with lush green grass.

When a beloved pet dies, the pet goes to this place.
There is always food and water and warm spring weather.
The old and frail pets are made young again.
Those who are maimed are made whole again and they play all day.
There is only one thing missing.

They are not with their special person who loved them on earth.
Each day they run and play until the day comes when one suddenly looks up.
The nose twitches! The ears are up!
The eyes are staring! And this one suddenly runs from the group!
You have been seen and when you and your special friend meet,
you take them in your arms and embrace.

Your face is kissed again and again,
and you look once more into the eyes of your trusted friend.
And you cross the Rainbow Bridge together,
never again to be separated.
