Adult is 16+ years and older. Run or walk our 5K with or without a dog! All registered runners receive a Tails on the Trail t-shirt. *There is a maximum of two dogs per registered adult participant. Youth is 6-15 years old. All registered runners receive a Tails on the Trail t-shirt. **Please note: children under 16 years old must be under direct supervision of an adult at all times and cannot walk a dog without an adult present. Children 5 & under: Animal lovers of all ages can join us at Tails on the Trail! Children 5 and under are free with a registered adult, but do not receive a T-shirt.

Please take a moment to read the following waiver.
As the participant and/or parent/legal guardian of the minor child listed above, and in consideration for allowing myself or my minor child to participate in the city's recreation program(s) and activities noted above, hereby acknowledge and agree as follows on behalf of myself and my child: 1. I have requested that I or my minor child be allowed to participate in the city's recreation program(s) and activities recognizing that such participation involves, or may involve, risks, both known and unknown, of physical injury or illness. 2. I represent that I and/or my child is physically capable of participating in the recreation program(s). 3. By signing this release, I,on behalf of myself and my minor child, expressly assume all risks, known and unknown, of injury, illness and property damage to myself, my minor child, or to any third party arising from or related to my or my child's participation in the city's recreation program(s), whether caused by the act, error, omission, or negligence of the city, its employees, officers or agents, or by an other person or cause. 4. By signing this release, I, on behalf of myself and my minor child, expressly exempt, waive, release and discharge in advance the Ark-Valley Humane Society, its employees, officers and agent, from any and all claims, liabilities, actions, or damages for injury, illness or loss that may arise from my or my child's participation in the city's recreation program(s) and activities, whether caused or created by the acts, errors, omissions, or negligence of the Ark-Valley Humane Society, its employees, officers or agents, or some other person or cause; and I further agree to hold harmless and indemnify the Ark-Valley Humane Society, its employees, officers and agents, from any and all injuries, damage, loss, claims or demands which arise from or are related to my or my child's participation in the city's recreation program(s) and activities. 5. I understand that this agreement incorporates the entire understanding and agreement between myself, my minor child, and Ark-Valley Humane Society, its officers, employees, agents and representatives, and that it cannot be modified or changed in any way by the statements, promises or representations of any employee or agent of the Ark-Valley Humane Society,; and that this agreement is intended to be as broad and inclusive as permitted by the laws of Colorado, and that if any portion is held invalid or unenforceable, the remaining portions shall continue in full legal force and effect. 6. By executing this agreement I also do hereby authorize the Ark-Valley Humane Society, its employees, officers and agents, to provide and/or consent to emergency medical or surgical examination and treatment for myself or my minor child in the event of injury or illness occurring to me or my child while participating in any recreational program or activity. 7. I hereby consent to being photographed participating in Ark-Valley Humane Society events, and to these photographs being used in marketing publication and online media. 8. My signature below indicates that I have read this document in its entirety, that I understand it completely, and that it affects my legal rights and the legal rights of my child, and that I, along with all heirs, assigns and personal representatives for myself and my child, agree to be bound by its terms. I also acknowledge that I am providing this waiver and release in advance for the benefit of the city knowing that all possible risks or causes of injury can neither be foreseen nor eliminated. I understand that participating in Ark-Valley Humane Society's (AVHS) Tails on the Trail event on May 19, 2019 includes an element of risk both to myself and the dog(s) I have entered. I should not participate unless both the dog(s) and I are physically able and properly trained. I agree to abide by any decision of an event official relative to my, and my dog's, ability to compete in this event safely and further agree that event officials may authorize necessary emergency treatment for me or the dog(s). I also understand that, although safety precautions including, course markers and road crossing volunteers will be provided, both vehicle traffic and spectators may be present along the course and I assume the risk of participating under such conditions. I further assume any and all other risks associated with participating in this event including, but not limited to illness, traveling to and from the event, falls, contact with spectators or other participants or dogs, the effects of weather (including temperature extremes and humidity) and the surface condition of the roads, sidewalks and trails. All such risks being understood and appreciated by me. I also affirm that the dog(s) I will be participating with has current Parvo and rabies vaccinations. I further agree to abide by all the rules of participation in this event. Having read this waiver and knowing these facts, and in consideration of the acceptance of my entry, I hereby for myself, my heirs, executors, administrators or anyone else who might claim on my behalf, covenant not to sue, and waive, release and discharge the Ark-Valley Humane Society, event officials, volunteers and any and all other sponsors, suppliers, agents, independent contractors, employees and any other of negligence or carelessness on the part of the persons or parties named in this waiver. I also understand and agree that AVHS and any sponsor may subsequently use, for publicity or promotional purposes, my name, information or image, without liability or obligation to me. Entries cannot be accepted without a valid signature. Entries from minors will only be accepted with a parent or legal guardian's signature.
