Pre-Adoption Contract
In order to finalize the adoption of the above listed animal, I understand that the animal must be spayed or neutered (unless waived for medical reasons by AVHS's veterinarian) and current on its rabies vaccination (unless under 12 weeks of age). I understand that this animal may be located in a foster home where pick up arrangements will be needed, and delays for pick up may occur. I understand that the animal I am putting a pre-adoption deposit on remains the property of AVHS until the finalization of the adoption on the date of pick up. (Please Initial)
Agreed Upon Pick-Up Date entered below. I agree to pick up this animal by 5:30 pm on the date entered below from AVHS, with the understanding that if the sterilization or vaccination was not performed due to any reasons prior to pick-up, AVHS retains the right to extend the pick-up time/date. If a date is not specified below, I agree to pick up this animal within 24 hours after being notified by AVHS by phone. If I do not pick up the animal, or make other arrangements with AVHS (719-395-2737) within 24 hours of the below stated date or time communicated to me by AVHS, I understand that AVHS will release the animal from this pre-adoption hold and it will be made available for adoption to the general public. I understand if this occurs, my adoption fee received by AVHS will not be refunded to me, but will be retained by AVHS.*
